So here’s my timeline:
Accrue 360 days of sea time in the 5 years leading up to expiration.
8 months prior to expiration, apply for renewal. (But no later than 1 year after expiration.)
Apply for and receive TWIC
TBD – Finished CPR / First Aid training
TBD – Completed Drug Test and Physical (expect Medical Card 3 months later)
TBD – Submitted Application Packet to the USCG
TBD – Received Mariner’s Medical Card in the mail
TBD – Received Captains License in the mail

The Details:
Sea Time requirements to renew the OUPV Near Coastal:
360 days on the water (4 hour minimum per day) within the past 5 years.
(Or open-book exercise)
(Or complete approved refresher training course)
(Or proof of working in certain instructor capacities)
Must include vessel documentation if you are the owner.
Otherwise, need the captains signature for each vessel served on.

TWIC:
Renewing the TWIC is exactly the same as getting an original.
Fill out a form online, make an appointment at your nearest application center, and show up with your Passport (or Driver’s License and Birth Certificate) and $128 fee.
They will collect fingerprints and a photo and mail the TWIC Card to you within a month.

Physical and Drug Test:
Same process as the original.
Search around for a medical clinic that does Mariner’s Physicals.
Medical Corner by the Honolulu Airport is very familiar with CG-719K and P.
Be sure to bring the printed forms ready to be filled out.

$90 for both the Physical and Drug Test.
The Drug test results will be sent directly to the Coast Guard, but keep the Physical paperwork for the application packet.
First Aid and CPR:
Same process as the original.
Red Cross has a program which works very smoothly. I completed the online training and then scheduled the hands-on portion, easy peasy. The certificate was emailed to me a few hours after completing the hands-on portion.

When handing in the application packet at the USCG station, they told me to pay online and email the receipt back. But it’s easier to just pay online and have the receipt ready to turn in then and there.

$50 for the “Renewal – Officer ” application to be evaluated, and
$45 for issuance of the certificate.
Total payment of $95 goes to Pay.gov https://www.pay.gov/public/form/start/4795779/, print the receipt, good to go.

Apply between 8 months prior to expiration to 1 year after expiration. Renewal will be issued with a post date matching your current MMC expiration.
